What is the full process for obtaining a medical marijuana card?

First, see if you qualify for a medical marijuana card. Then you must be diagnosed with a debilitating or chronic condition and submit the following requirements to the Department (LARA-Michigan)

• A patient form with the complete required information
• A physician certification form that has been approved, signed by your licensed physician.
• The required fee. Make checks or money orders payable to: State of Michigan-Medical Marihuana Program (MMMP)
• You may optionally designate a caregiver. Qualifying patients 18 years of age or older are not required to designate a caregiver.
